Important Forms

Before travelling overseas, it is very important to aquire and complete the following forms. All forms can be found at the OIP office located in Thomas Hall, 203.

• Budget This is a rough estimate of what you are going to spend on your trip. Often times university websites have a section on their webpage that will help with this. Complete this form and review it with Peg Mason in Financial-Aid.
• Information Release Form and Health Insurance This insures that you understand that you are embarking on this journey on your free will and CSM is not responsible for accidents or injuries that may occur while you are overseas.
• Travel Waiver A statement stating that you understand that you are entering a new culture and you understand the risks.
